这是一种发病率最高的龟头包皮疾病。患者几乎都有包茎或包皮过长。在龟头和过长的包皮之间，由于脱落的上皮细胞、腺体分泌和包皮垢杆菌形成一个温热、潮湿的细菌培养基，一旦细菌进入即可引起炎症。
阴茎头发炎和包皮发炎常常同时存在，故统称为阴茎头包皮炎。引起阴茎头包皮炎的主要原因是包茎或包皮过长。另外，不洁性交、药物刺激或过敏，也是致病的原因。
阴茎头包皮炎表现为阴茎头及包皮红肿、疼痛、奇痒，部分可出现糜烂及溃疡，并有黄色脓性分泌物，伴有特殊臭味。严重时可出现阴茎头坏死。部分病人在急性期后发生尿道口粘连狭窄，引起排尿困难。反复感染可使阴茎头或包皮增厚，形成白斑。
服用某些药物过敏时，有时可表现在阴茎头或包皮上形成固定的水肿性红斑，中心部位出现水泡，破溃后可引起感染。症状多在服药后24-36小时内发生，停药后好转，再次服药又重复出现。
阴茎头包皮炎在急性发作期应将包皮上翻，可用1:5000高锰酸钾溶液洗涤，然后涂抗生素软膏。如包皮不能上翻者，应作切开引流。在进行局部处理的同时，需要口服或注射抗生素。由药物过敏引起的还应服用抗过敏药物。
