慢性肾炎一部分是由急性肾炎发展而来，但比例较小，大部分患者起病隐袭，病因不明。临床上就诊的患者病程可长可短，有的病人过去没有任何肾炎病史，几天内突然出现蛋白尿、镜下血尿、水肿、高血压等症状。基层医院多数误诊为急性肾炎，实际上一发病就属于慢性肾炎。慢性肾炎在病程或发病方式上有以下几种特点：
（1）急性肾炎起病未能彻底治愈，临床症状及蛋白尿、镜下血尿持续存在，迁延一年以上而演变为慢性肾炎。这部分患者约占急性肾炎的10％左右。
（2）过去有急性肾炎病史，当时已临床治愈，经过相当长的时间（数年甚至10～20年），因感染、过劳、上呼吸道感染等，又出现蛋白尿、水肿、高血压等肾炎症状。
（3）过去没有任何肾炎病史，短期内在某些诱因或无诱因情况下，出现蛋白尿或血尿等症状，甚至出现水肿或血压增高等，经适当休息和积极治疗后，部分患者症状减轻，但有的患者因感染、劳累而反复发作。
（4）既往无肾炎病史，发病时即为肾功能不全、贫血等，有的病人甚至肌酥、尿素氮很高，已是尿毒症期，经检查才发现有蛋白尿、血尿等异常，或已有低比重、比重固定尿，而诊断为慢性肾炎伴肾功能不全。
慢性肾炎的发病形式及病程特点，从另一个侧面反映出定期健康检查的重要性。
