肾功能不全时，选择使用抗生素的一般原则是：

①无明显感染迹象者，不给予抗生素；

②有感染者，应根据培养及药敏试验选择抗生素；

③选择对肾脏无毒或毒性较小和不因药物蓄积而产生严重损害其他器官的抗生素。

关于药物剂量的调整，应注意以下几点：

（1）基本不需要调整剂量的抗生素：红霉素、强力霉素、利福平等。

（2）需要适当调整剂量的抗生素：林可霉素、青霉素族。氯霉素、两性霉素B等。通常约减少到正常量的1／2～1／3。

（3）必须减少剂量的抗生素：氨基糖式类抗生素、妥布霉素、多粘菌素及先锋霉素。

（4）不宜应用的抗生素：先锋霉素正、四环素、万古霉素、新霉素。
